New Patient Registration
If you wish to register as a patient with the practice, you must live within our practice area, provide photographic ID and proof of address. You will therefore need to attend in person and complete the relevant patient registration form as well as a questionnaire about health related matters. We are unable to provide online registration.
When you register with us we will arrange for you to attend our treatment room for a " mini medical" with the nurse. We will also give you an appointment to see a doctor.
Please note if you are from abroad further special paper work may be required to confirm your elegibility for NHS treatment.
We provide General Medical Services as per 2004 GP Contract and a range of enhanced services as specified.
